Before completing the online application, be sure to have a PDF copy of any equipment bids, quotes, or other items you wish to upload as part of the application. Also, have a separate PDF document with a current IRS Form W-9 for your organization to upload. Applications without the appropriate attachments will not be accepted and you will need to complete the application form in its entirety once again.
